ABSTRACT

Introdução: a causa da unha em gancho é o trauma do hiponíquio. A lâmina ungueal se apresenta com hipercurvatura longitudinal de concavidade volar, causando perda funcional, comprometimento estético e dor. Objetivo: avaliar os resultados na pesquisa de 20 prontuários de pacientes submetidos a cirurgias de correções de unhas em gancho de etiologias traumáticas. Métodos: estudo retrospectivo transversal epidemiológico, de 2010 a 2018, de prontuários de pacientes submetidos à técnica cirúrgica de Bakhach, no ambulatório de Cirurgia da Mão do Hospital do Servidor Público Municipal de São Paulo. Resultados: 20 dedos acometidos. Sete vítimas de acidente com objetos cortantes, cinco com máquinas tipo prensa, cinco com portas, um com janela, um com motocicleta e um por mordida de cachorro. O dedo com maior incidência foi o médio (12 casos); seguido do indicador (cinco casos) e do anelar (três casos). A queixa principal foi estética (11); dor (seis) e funcional (três). Todos tiveram amputações digitais distais transversas do tipo II e foram submetidos a tratamento cirúrgico. A cirurgia de reconstrução ocorreu entre quatro e 25 meses pós-trauma. Conclusão: 15 ficaram satisfeitos, embora dois destes tenham relatado dor: um na articulação interfalângica distal e o outro no hiponíquio. O acompanhamento variou de seis meses a dois anos.


Introduction: The cause of the hook nail is the trauma of the hyponychium. The nail plate presents longitudinal hypercurvature of volar concavity, causing functional loss, aesthetics concerns, and pain. Objective: To evaluate the research results of 20 medical records of patients submitted to hook nail correction surgeries of traumatic etiologies. Methods: Epidemiological, cross-sectional, retrospective study of patients' medical records who were submitted to Bakhach's surgical technique, from 2010 to 2018, in the Hand Surgery Outpatient Clinic of the Hospital do Servidor Público Municipal de São Paulo. Results: We analyzed 20 affected fingers: 7 were victims of accidents with sharp objects, 5 with press machines, 5 with doors, 1 with window, 1 with motorcycle, and 1 due to a dog bite. The finger with the highest incidence was the middle finger (n=12), followed by the index finger (n=5), and ring finger (n=3). The main complaint was aesthetics (n=11); pain (n=6), and functional (n=3). All cases had type II distal digital transverse amputations and underwent surgical treatment. The reconstruction surgery occurred between 4 and 25 months after the trauma. Conclusion: 15 patients were satisfied and two felt pain: one in the distal interphalangeal joint and the other in the hyponychium. The follow-up ranged from 6 months to 2 years

ABSTRACT

A deformidade da unha em pinça é caracterizada por excessiva curvatura transversal da unha. Uma mulher de 58 anos apresentou-se com queixa de unha em pinça acompanhada de edema e dor no último 1 mês devido ao uso frequente de sapatos estreitos. Foi realizada uma cirurgia para tratar dor, inflamação, interferência no uso de calçados e cosmético, utilizando o método de incisão em T invertido com remoção de osteófitos. Este caso mostrou que a abordagem cirúrgica não foi demorada, apresentando curto tempo de cicatrização, dor mínima e excelente resultado cosmético.


Pincer nail deformity is characterized by excessive transverse over curvature of the nail. A 58-year-old woman came with a complaint of pincer nail accompanied by swelling and pain in the last 1 month due to frequent use of narrow shoes. We performed a surgical intervention indicated to treat pain, inflammation, interference with wearing shoes, and cosmetic embarrassment using an inverted T incision method with osteophyte removal. This case showed that the surgical approach did not require a long time to be conducted, presenting brief healing time, minimal pain, and excellent cosmetic outcome.

Pincer nails are a kind of nail deformity characterized by abnormal thickening and distal transverse overcurvature of the nail plate,whose etiology and pathogenesis are still unclear.According to the morphology of the nail plate,pincer nails can be divided into three subtypes:common pincer nail (trumpet nail),tile-shaped nail,and plicated nail.Width index,height index and curvature index are objective indicators to evaluate the severity and therapeutic effect of the pincer nails.According to the classification and severity of the pincer nails,appropriate treatment methods can be adopted to improve the appearance of the nail plate and relieve clinical symptoms of patients.This review systematically summarizes the classification of,evaluation of the severity of and several therapeutic methods for pincer nails.

ABSTRACT

Secondary nail deformities are often seen at some time after fingertip and nail bed injuries. A proper evaluation of the fingernail enables appropriate diagnosis and treatment, especially in patients who need surgical management. To date, ultrasound and magnetic resonance imaging are the only diagnostic tools that most surgeons use in cases of nail deformities. Herein, we report the case of a patient with a nail deformity who underwent successful correction based on the preoperative use of real-time optical coherence tomography.

ABSTRACT

Abstract: The nail-patella syndrome involves a clinical tetrad of changes in the nails, knees, elbows and the presence of iliac horns. Nail changes are the most constant feature: absent, hypoplastic, or dystrophic. A pathognomonic finding is the presence of the triangular lunula. The diagnosis of nail-patella syndrome is based on clinical findings. In this paper we will discuss a case report of this syndrome and its relation with a dermatological finding.

ABSTRACT

Abstract The Iso-Kikuchi Syndrome is a rare condition characterized by nail dysplasia involving the index fingers, including micronychia, polyonychia, anonychia, irregular lunula, malalignment and hemionychogryphosis. On the antero-posterior image, radiologic examination reveals a narrowing of the distal phalanx. The lateral image shows a Y-shaped bifurcation of the distal phalanx. We report a case of a patient with typical clinical and radiologic signs of Iso-Kikuchi Syndrome.

ABSTRACT

Os onicomatricomas são tumores benignos raros que se originam a partir da matriz ungueal e do estroma subjacente. São geralmente assintomáticos e de crescimento lento. Acometem igualmente homens e mulheres de meia idade, comprometendo com maior frequência os dígitos das mãos. As características clínicas fundamentais para o diagnóstico são: faixa longitudinal amarelada de espessura variável, estilhaços hemorrágicos, estrias longitudinais associadas à hipercurvatura transversal e projeções digitiformes emergentes da matriz ungueal. Os autores relatam três casos desse tumor acometendo pododáctilos, enfatizando seus principais aspectos clínicos, achados dermatoscópicos e tratamento cirúrgico.


Onychomatricomas are rare benign tumors originating from the nail matrix and underlying stroma. They are usually asymptomatic and slow growing, affecting both middle-aged men and women, and more frequently involve the digits of the hands. Key clinical features for diagnosis are: yellowish longitudinal band of variable width, splinter hemorrhages, longitudinal grooves associated with the transverse overcurvature and fingerlike projections emerging from the nail matrix. The authors report three cases of this tumor affecting toes, emphasizing main clinical aspects, dermoscopic findings, and surgical treatment.

O pinçamento do leito ungueal gerado pela hipercurvatura transversa da unha é chamado de unha em pinça e pode provocar dor e constituir problema estético. A unha em pinça pode apresentar três tipos clínicos, mais comuns, sendo a unha em telha um deles. As causas podem ser hereditárias ou adquiridas. Os dedos dos pés são acometidos com maior freqüência. Os tratamentos conservadores são os preferidos pelos pacientes, porém, os tratamentos cirúrgicos produzem os melhores resultados. Das técnicas cirúrgicas, a técnica de Zook é opção diferente, pois, preserva a matriz e o tamanho da unha. Os autores apresentam cinco pacientes com unha em telha, tratados com a técnica de Zook.


The pinching of the distal nail bed, caused by transverse overcurvature of the nail is called pincer nail. It frequently causes pain and aesthetic problems. Pincer nail may present in three frequent clinical types, one being the tile-shaped nail. The causes may be hereditary or acquired. The toes are most frequently affected. Conservative treatments are preferred by patients, although surgical treatments produce better, longer-lasting results. Among the surgical techniques, Zook’s technique differs from others, because it preserves the matrix and consequently the size of nail. The authors evaluated five patients who presented pincer nails and were treated using Zook’s technique.

ABSTRACT

A síndrome das unhas frágeis é queixa comum, caracterizada por aumento da fragilidade das lâminas ungueais. Afeta quase 20 por cento da população geral, sendo mais comum em mulheres. Clinicamente se manifesta com onicosquizia e onicorrexe - distúrbios nos fatores de adesão intercelular das unhas se manifestam como a primeira, ao passo que alterações da matriz apresentamse com onicorrexe. Mesmo sendo tão usual e afetando os pacientes de maneira importante em seu cotidiano, o tratamento das unhas frágeis avançou pouco nas últimas décadas e ainda se baseia principalmente no uso da biotina.


Brittle nail syndrome is a common condition, characterized by increased fragility of the nail plates. It affects almost 20 percent of the population, being more usual in women. Clinical manifestations of brittle nails are onychoschizia and onychorexis - disorders of intercellular adhesive factors are expressed as the first, while disorders of the nail matrix manifest as onychorexis. Despite being so common and causing much more than only cosmetic problems to the patient, the treatment of brittle nails has had little improvement over the past decades and is still mainly based on the daily use of biotin.
